Service Coordination Tool Kit

As a result of amendments to Public Health Law enacted in 2012, that became effective in January and April, 2013, service coordinators in the Early Intervention Program (EIP) have expanded responsibilities. In an effort to assist service coordinators with the performance of these duties, the Department is pleased to provide the Service Coordination Tool Kit attached below.

The Service Coordination Tool Kit includes the following information:

  • An overview of new service coordination responsibilities in law  (PDF)
  • An Insurance Tool Kit including information and templates for collection of insurance information, consent/release of information, etc.  (PDF)
  • A Transition Tool Kit which has been updated to reflect new service coordinator responsibilities (PDF)
  • Information on Individualized Family Service Plan (IFSP) Implementation (PDF)
  • Information on Office for People with Developmental Disabilities (OPWDD) Notification  (PDF)

The Service Coordination Tool Kit is very comprehensive and details the procedures service coordinators should follow while performing their responsibilities in the EIP. Service coordinators should begin utilizing the information and the standardized templates contained in this tool kit immediately.
