COVID-19 Vaccine Information for Early Intervention Providers Updated January 13, 2021


Dear Colleague,

Early Invervention (EI) therapists and teachers with active service authorizations are eligible for COVID-19 vaccination. Providers should follow the guidance provided through the links below to complete the eligibility questions, confirm your eligibility, and schedule an appointment to get the vaccine. Vaccine distribution is contingent on availability.

To confirm whether you are eligible for the COVID-19 vaccine: https://forms.ny.gov/s3/vaccine

Additionally, the Department’s website notes that individuals being vaccinated must bring proof of eligibility to the vaccination site, and states that this may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status.

An independent EI provider who wishes to identify themselves as such and their profession, may show their current, approved Provider Agreement to establish their identity as an approved EI provider in NYS.
