Revised Insurance Tool Kit for Service Coordinators Available - Correction

Please note: Form F was removed from the toolkit. This has been corrected in the memo below.

The Bureau of Early Intervention (BEI) is pleased to announce that the Insurance Tool Kit for Service Coordinators has been revised to comport with regulations adopted on December 5, 2018. This update included removing “Form F – Consent to Bill Non-regulated Insurance” as regulation changes no longer allow the Early Intervention Program to use insurance that is not regulated by New York State Insurance Law.

The revised document and updated fillable forms are available on the New York State Department of Health Bureau of Early Intervention webpage through the following link: https://www.health.ny.gov/community/infants_children/early_intervention/memoranda.htm. Please note that BEI is in the process of updating the non-English versions of the Child Insurance Information Form B/Instructions and Parent Notice Regarding Insurance, as well as translating Form C/Authorization To Release Health Insurance Information. Further communication will be sent when the translation has been completed and the documents have been posted.
