Revised Guidance Document: Individualized Family Service Plans

The New York State Department of Health is pleased to provide the revised  Early Intervention Program Guidance Document, Individualized Family Service Plans.

This guidance document has been revised based on changes in federal regulations and recent revisions to state regulations for the Early Intervention Program adopted on November 30, 2016.
