In accordance with Federal Law, Providers who received payments for Early Intervention Services in the 2014 calendar year should expect to receive a number of 1099 forms as noted below. Those forms will be mailed to Providers on or before January 31, 2015.
Providers will receive a 1099 from New York State Department of Health via the State Fiscal Agent (SFA), for amounts from the State Escrow account in 2014 (*see details below), a 1099 from Medicaid, and a 1099 from each insurance company (**see details below).
The Department and the SFA have informed insurance companies and Medicaid that they need to create a 1099 for billing Providers. During the 1099 creation process, there are procedures in place that help validate the accuracy of Provider information on 1099s such as Provider Name and Tax Identification Number (TIN) with the IRS. During the 1099 creation process, if there are any errors that prevent a 1099 from being issued to a Provider, the Provider will immediately be contacted by the SFA to verify and resolve any data discrepancies.
*The SFA will not issue a 1099 to a Provider that received less than $600 in payments from the State Escrow account during the 2014 calendar year.
