Service Coordination Tool Kit Reissued

Colleague:
As a result of amendments to Public Health Law enacted in 2012, that became
effective in January and April, 2013, service coordinators in the Early
Intervention Program (EIP) have expanded responsibilities. In an effort to
assist service coordinators with the performance of these duties, the
Department issued the Service Coordination Tool Kit on April 26, 2013.
Based on
feedback from the EIP municipal and provider community, the Service
Coordination Tool Kit has been updated and improved.
Please find
attached to this e-mail the following sections of the Service Coordination Tool

The Service
Coordination Tool Kit is very comprehensive and details the procedures service
coordinators should follow while performing their responsibilities in the EIP.
Service coordinators should begin utilizing the guidance and the standardized
templates contained in this tool kit immediately.
The
Insurance Tool Kit is still in the revision process and will be reissued in the
near future. This tool kit will include expanded guidance and improved
templates for collection of insurance information, consent/release of
information, etc.
